The VCO systeme entails absolutely no modifications, neither in the present nor in the traditional technique of osteotomy. The same goes for the fibula, where a low osteotomy can be realised just like the deinsertion of superior fibula tibial. A first pin marked, parallel to the interline is place as laterally as possible at the level of the head of the fibula. This pin will have to be located in the cortical without going through. The mark on the pin can be seen outside the eyepiece. The second pin is placed in order to stabilise the whole. From then on, two cuts can be realised. The first, always parallel to the interline, according to the chosen correction. A side cut impression of two centimetres on the external face and anterior posterior is sufficient, the osteotemy can be finished in safety after the ablation of the visor. It is advised on the first horizontal cut to stop at 1 cm before the opposite cortical. The osteotomy thus realised, the placing of the VCO system can be carried out.

After having exposed the lateral face to the extremity of the tibial, we remove with care the tibial periost without opening the articulation. The use of a brillance amplifier is recommended to control the different stages. The cutting angle and the osteotomy can be realised through an eyepiece with modular right or left cut.
