In the case where the osteotomy has provoked an internal and unstable splinter, it is possible before realising a too strong compression, to place the superior screw at an oblic angle of 45° from the plane of the housing of the osteotomy, taking the opposite cortical . This technique can be also applied in case of retaking of osteotomy, and in this way ensuring a perfect stability (internal pseudo arthritis for example). A plate for additional osteotomy is also available. Its use remains identical. By its minimal blockage it enables to answer to all possible cases. It enables also a progressive distraction of the osteotomy and a slight compression in order to stabilise t he whole. An internal VCO can also be placed before realising an osteotomy, avoiding in this way a somewhat derotation. To the two mostly used techniques which are tibia osteotomy of valgisation for external subtraction or by internal addition, can be added the osteotomy of valgisation curvilinear and of derotation. The VCO system also applies for inferior femoral osteotomy for varisation.

The re-education is immediate.

The bone consolidation is obtained between 4 and 6 weeks, which enables a quick functionnal result.
