Antero-posterior femoral measurement

This gauge allows the antero-posterior size of the distal end of the femur to be assessed. A suitable femoral prothesis is chosen, by finding the intersection point between the most proximal part of the anterior edge of the prosthesis, and the anterior cortical of the femur. A probe is used to find the place where the femoral prosthesis, whatever its size will meet the anterior cortex of the femur. The probe is placed onto the gauge, according to the size of the measurement aimer used (1,2,3,4 or 5). The exact point where the prothesis will seat is therefore assessed : if the probe, which is positionned on the anterior cortical bone, overlaps by more than 2 mm, the aimer of the next size up should be chosen. If it is more than 2 mm away, the next size down should be used. Once the operation is completed, the measurement viewer is placed onto the resected area of the distal end of the femur, in a neutral rotational position, using the posterior condyles for guidance.

For resection of the anterior, posterior and bevelled surfaces of the femur in a neutral direction (without correcting external rotation), the two posterior holes of the A/P measurement aimer should be used, without taking into account the medial and lateral marker spikes of the gauge. The distal resection is marked with two center punches through the holes of the A/P guide. The cor- responding 4 resection guide is placed into these marks. The anterior and posterior resections, as well as the two bevelled resections are carried out using the gauge corresponding to the A/P guide.
